Needs Assessment

The Coalition for a Healthy NJ completed a Comprehensive Community Health Needs Assessment (CCHNA) titled “Health in Focus: Assessing Factors of Health in NJ,” in order to identify the needs, gaps and resources that impact chronic disease within a 13-county targeted area of New Jersey.  This area was defined by the Community Transformation Grant.
